## Shoulder soreness often starts with extra work

Scottsdale’s cooler mornings can bring more outdoor time. Added reaching or carrying may leave your shoulder sore. That doesn’t always mean a serious injury.

Four small muscles wrap around your shoulder joint. A tendon is a tough cord connecting muscle to bone. Either the cords or the joint may become sore.

## The painful movement can point toward the cause

Those four muscles and cords form the rotator cuff. They lift your arm and hold the joint steady. You’ll often feel soreness during an overhead reach.

Joint wear often feels deep and stiff. It’ll sometimes bother you after sitting still. Your shoulder may also grind or move less.

A frozen shoulder grows tighter over time. It’ll stay stiff when a doctor moves your arm. Pushing harder won’t loosen that tight joint.

A cord can tear slowly or during a fall. You may still use your arm with a slow tear. Sudden weakness after a fall needs a prompt exam.

## Gentle changes can ease soreness at home

Don’t repeat the reach that wakes the ache. Move your arm only where it feels comfortable. Easy motion shouldn’t leave you worse for hours.

Support your arm on a pillow during sleep. Don’t lie on the sore shoulder. If sleep breaks again, move the pillow or turn over.

Check the shoulder that evening and after sleep. It should return near its usual soreness. If it doesn’t, reduce the next reach or lift.

Change only the distance or the weight first. Don’t make both harder on the same day. You’ll know which change your shoulder handled.

You’ll notice changes during dressing and washing. Those tasks shouldn’t keep getting harder. Arrange an exam if motion or strength keeps falling.

## Scottsdale outings can add shoulder work

The McDowell Sonoran Preserve has over 230 miles of shared-use trail. Carrying water and handling straps can tire your shoulder. Golf and court play add turning and overhead reaches.

January through March often brings more activity. Don’t increase every shoulder task together. Let soreness after the next night’s sleep guide you.

1. In the MOON Shoulder prospective cohort of 452 patients with symptomatic, ATRAUMATIC full-thickness rotator cuff tears, physical therapy succeeded in more than 70% of patients at 10 years: only 115 (27.0%) had surgery at any point over the decade. Patient-reported outcomes improved with physical therapy and did NOT decline over 10 years in those who never had surgery. The strongest predictor of early surgery was low patient expectation of physical therapy, not tear anatomy.

2. A 2025 Bayesian network meta-analysis of exercise for rotator cuff-related shoulder pain (15-16 studies, 913-947 participants) ranked CONCENTRIC strengthening training first for both pain and shoulder dysfunction; eccentric and traditional training were moderately effective and significantly better than motor-control and scapula-focused training.

3. The Cochrane review of manual therapy and exercise for rotator cuff disease included 60 trials and 3,620 participants but could perform no meta-analysis because of clinical heterogeneity and incomplete outcome reporting; risk of bias was high in 43 of the 60 trials. The single high-quality placebo-controlled trial (120 participants) found manual therapy plus exercise no better than inactive ultrasound at 22 weeks on overall pain.

4. A UK primary-care database study of 658,469 adults found an annual prevalence of consulting for a shoulder condition of 2.36% and an incidence of 1.47%. Over three years of follow-up, about half of incident cases consulted only once, 13.6% were still consulting in year three, 22.4% were referred to secondary care and 10.6% received an injection from their GP. Five of 426 possible diagnostic codes accounted for 74.6% of new-case diagnoses.

## Take your shoulder questions to the visit

If soreness hasn’t settled, note how it started. Include lost motion, weak reaches, and poor sleep. Those details will help during your exam.

Ask which choices fit your shoulder. Ask about cost and return visits, too. Don’t leave until you understand what happens next.
